Attention Traveling Eucharistic Ministers - the Diocese of Allentown requires you to complete 3 steps for insurance liability reasons - A Motor Vehicle Record Check, Driver Information Form, and to watch the Catholic Mutual Group informational video "Be Smart-Drive Safe". Above please find a PDF containing the Diocese of Allentown Transportation Policy, as well as the Motor Vehicle Record Check Form and Driver Information Form. If you are a visiting Eucharistic Minister, please fill out both of these forms and return to the Parish Safe Environment Coordinator as soon as possible. We will submit the forms to the Diocese of Allentown, and get you set up to watch the informational video online. 

Forms can be submitted electronically via janessa.odonnell@stjoescoopersburg.org; in a sealed envelope addressed to PARISH SAFE ENVIRONMENT COORDINATOR, which can be placed in the offertory basket at any weekend Mass; personally delivered to the Rectory during normal business hours (typically, Monday-Friday 10AM-3PM); or, mailed to the Rectory at 5050 St. Joseph's Road, Coopersburg, PA 18036.
